2010 ABC Individual Player Registration Form

1. Click on the ABC Registration Form link listed above and print a form for each child whom you would like to register.
2. Completely fill in a form for each participant.  Be certain to make us aware of any medical conditions.
3. To determine which age group your child can play in, it is his/her age on April 30th, 2011.
4. If you have TWINS or more and want them on the same team, please make a SPECIAL REQUEST at the bottom of each form.
5. All other SPECIAL REQUESTS will be considered, but none are guaranteed to be honored.
6. Get to one of the registration dates on the League Calendar or below with the following items:
  • Registration Form for each participant.
  • $45 check, cash, or money order attached to each Registration Form.  Separate checks or money orders are not necessary for multiple participants.
  • If a first-time participant, please bring a copy of his/her birth certificate for ABC to keep on file.

  • Thu, Aug 12th @ Oak Grove Community Center from 6pm - 8pm
  • Sat, Aug 14th @ St. Amant Park Rec Center from 9am - 12pm
  • Wed, Aug 18th @ Geismar Community Center from 6pm - 8pm
  • Sat, Aug 21st @ Oak Grove Community Center from 9am - 12pm
8. Turn in your form to any ABC volunteer and be certain to receive an information sheet for your appropriate age league(s).
  • NOTE: Registration Forms will NOT be accepted without full payment.
  • NOTE: Once your paperwork is received, NO REFUNDS are issued unless there is some reason we cannot place your child(ren) on a team.
 

2010 TOURNAMENT TEAM REGISTRATION Information

1. Must be a registered team with USSSA.
2. Must have team insurance.
3. Must register through the USSSA website.

5. Once registered, no REFUNDS are issued unless we cannot place your team in a league.
